Fort Memorial Hospital Foundation scholarship applications now available

FORT ATKINSON – Fort Memorial Hospital Foundation is accepting scholarship applications from students pursuing health careers during the 2015-2016 academic school year. Scholarships valued at $500 to $5,000 each are available for students at the undergrad and graduate levels. Applications must be submitted online through the Fort Scholarships website, operated by the Fort Atkinson Community Foundation, by February 15.

A number of scholarships offered by the Fort Memorial Hospital Foundation are listed on the Fort Scholarships website. They are:

The Scott W. Hulstein Scholarship was established in his memory by his parents, Gerrit and Rebecca Hulstein. Applicants can be a high school senior or currently enrolled as an undergraduate at a college or technical school pursuing a degree in the health care field and must reside in Jefferson County. One $750 scholarship is available.

Cynthia McKinley Kolasinski Scholarships are awarded to students studying medicine or nursing at an accredited institution of higher learning. According to the trust established by Ms. Kolasinski, preference will be given to medical students beyond their second year and nursing students beyond their first year. Three $600 scholarships are available.

The Fort Memorial Medical Scholarship is available to residents of Jefferson County pursuing a degree in a health care field at a technical school, college or university in Wisconsin. Scholarship applicants can be high school seniors or undergraduate students who will be enrolled in the 2015 fall semester at an accredited medical school, Baccalaureate, Masters or Associate Degree program studying a medical or health sciences field. Up to $5,000 is available, but the actual award amount is dependent on the number of qualified applicants. Applicants must demonstrate a financial need.

The Estelle Stinson Chase & Peter A. Chase Scholarships are awarded to Fort HealthCare employees who are studying nursing (LPN, ADN, BSN Completion or MSN) at an accredited college or university. Consideration is given to the applicant’s service and involvement in the community and to Fort HealthCare, excluding job related activities. Three $500 scholarships are available.

The Williams Scholarship was donated by Dr. Donald and Barbara Williams in recognition of the important role Fort HealthCare employees play in providing excellent care for the people we serve. Applicants must be employees of Fort HealthCare studying in fields related to the healthcare industry.

For all Fort Memorial Hospital Foundation scholarships, qualified applicants must be accepted at an accredited institution prior to scholarship distribution.
