Workplace Wellness Awards

The purpose of the wellness seal of approval is to support Fort HealthCare's mission to "Improve the Health and Well-being of Our Community, and vision to "Be the Healthiest Community in Wisconsin. The award will be available to corporations (including schools), local organizations, and the faith community.

Company Information


Levels of the Award

  • Gold Star (comprehensive, results-driven program)
    Applicants will demonstrate an established program with positive participation and results for a minimum of two years. 100 points
  • Gold
    Applicants will demonstrate all of silver requirements plus two applications from each of the six dimensions of wellness. 90 points
  • Silver (basic program)
    Applicants will have in place a health risk assessment questionnaire, biometric data collection, a diverse wellness committee, a mission and vision of wellness documented. 80 points

Senior Level Support

Does your company have a written and posted mission and vision for wellness?
Do your CEO, COO, CFO, board members, and/or other senior level management participate in wellness programming and screenings both verbally and visibly?
Does your middle management communicate wellness initiatives and participate in offerings?

Company Support

Does your company encourage participation in wellness screenings?
Does your company incentivize for participation in a wellness screenings?
Is there a person designated as the wellness leader as part of his/her job duties?
Does your company have a wellness team or wellness committee?

Data Collection

Does your company offer a health risk questionnaire/assessment to employees on an annual basis?
Does your company offer a biometric screening to employees on an annual basis (including lab work for lipid panel and glucose, BMI, waist circumference, weight, and blood pressure)?
Does your company offer feedback and goal setting sessions to employees to review health risk and biometric results?
Does your company provide incentives for participation in a health risk questionnaire/assessment to employees?
What level of participation does your company have in the health risk questionnaire and biometric data collection?

Health Plan

Does your health plan cover preventive screenings such as Pap and pelvic, mammogram, colonoscopy, PSA, and bone density?
Does your health plan cover recommended immunizations?
Does your health plan offer mental and behavioral health coverage?
Does your company offer extra in house screenings such as; blood pressure, glucose, lipid panel, skin cancer or bone density?

Dimensions of Wellness

Social

Does your company support and participate in local events such as Relay for Life, Tomorrow's Hope, American Heart Association Walk?
Does your company offer volunteer opportunities such as highway clean-up, charity fundraisers or food drives?
Does your company offer a break area away from employee workstations?
Are employees at your company encouraged to take entitled breaks during their shifts?

Occupational / Environmental

Does your company have a recycling program?
Does your company offer ergonomic assessments?
Does your company provide for supplies and/or tools necessary to create an ergonomically correct workstation?
Does your company have a safety committee?
Does your company provide a private lactation room?

Spiritual (Mind / Body Connection)

Does your company offer a quiet room for relaxation?
Does your company offer flexible break times?
Does your company allow for music in designated areas?
Does your company allow personal items to be displayed at workstations?

Intellectual

Does your company communicate its goals and strategic plan?
Does your company empower employees by allowing them to be decision makers in their department and the company?
Does your company offer a suggestion box or a way for employees to voice their suggestions, concerns and praises?
Does your company provide reimbursement for educational courses taken through a credible institution?
Does your company provide reimbursement for educational classes taken in the areas of wellness such as classes in fitness, nutrition, stress management, work/life balance?
Does your company offer educational presentations for employees such as lunch and learns, webinars, or onsite workshops related to wellness?

Emotional

Does your company offer an EAP service?
Does your company offer flex scheduling when appropriate?
Does your company provide a daycare?

Physical

Does your company offer wellness challenges throughout the year?
Does your company offer onsite exercise classes or has an exercise area?
Does your company have a mapped and measured walking route?
Does your company offer reimbursement for fitness memberships?
Does your company provide healthy options in vending machines and/or cafeterias?

Wellness Activities and Program Participation

Does your company survey employees annually to assess needs and interest levels of employees?
What level of participation does your company have in its wellness activities and programs?
Does your company provide incentives for participation in wellness activities and programs?
Is there an annual evaluation of your company's wellness programming efforts?
