Wellness Phase of Recovery

Don’t stop exercising once you’ve finished your cardiac or pulmonary rehab program! Make what you learned through your recovery a regular part of your life. The Fort HealthCare Cardiopulmonary Exercise & Wellness phase is an affordable self-pay program that is open to anyone who has approval from a physician to exercise. We can also help you obtain approval from your primary care physician.

After the first two phases of rehab, a conditioning exercise program with new goals is developed. Or if you are new to exercise, the staff will help you develop an exercise program. Maintaining the exercise program becomes your responsibility, and we continue to support you on your wellness journey. The goal of this phase is to establish positive, exercise-conscious lifestyle changes.

Membership

With a physician-approved membership, you can take advantage of:

  • Access to our aerobic and strengthening equipment
  • Professionally-trained staff on hand including a nurse, exercise physiologist; and a respiratory therapist (for pulmonary rehab patients)
  • Staff who will monitor your blood pressure, heart rate, and oxygen saturation at each visit (supplemental oxygen is available if needed)

How Do I Enroll?

All you need to do to maintain membership in the Fort HealthCare Cardiopulmonary Exercise & Wellness program is obtain a physician referral (clearance to exercise), and schedule your payments and workout times.
