We need to practice social distancing both during exercise and while people are arriving and leaving the gym area. EVERYONE will:
In order to control the number of people in our gym area at any given time, we need to know who is going to be here and what time they plan on coming. We also ask you DO NOT bring guests into the gym with you.
We have scattered times available throughout the day – Monday, Wednesday and Fridays will begin at 6:30 a.m. and Tuesdays and Thursdays at 7:00 a.m. Please call us at 920-568-5349 to set up your time, you will have one hour of scheduled exercise. We CANNOT accommodate walk-ins at this time.
The Fort HealthCare Cardiac Rehabilitation program is designed to help an individual with heart disease return to an optimal level of physical, emotional, social, and vocational well-being. Under the direction of hospitalists that see patients at Fort Memorial Hospital, a cardiac rehabilitation team helps you achieve maximum benefit from the Cardiac Rehab Program. Your plan of care will be individualized to your needs and may include healthcare services from pharmacy, food service, rehabilitation services, Behavioral Health professionals, and other areas of care if needed.
The Fort HealthCare Pulmonary Rehabilitation program is an individually-tailored program featuring learning through exercise and education, which will help you decrease hospital stays and have more energy. This program is managed under the direction of a Fort HealthCare provider and includes a respiratory therapist, exercise therapist, exercise physiologist, registered nurse, registered dietitian, your personal healthcare provider, psychologist, and occupational therapist.
Our team will work with you and your personal healthcare provider to address any ongoing concerns and enable you to live a more active and fulfilling lifestyle.
