Your health is a team effort at the Jefferson Clinic.

We firmly believe in healthcare based on partnership: between Fort HealthCare and the communities we serve, as well as partnership between doctor and patient. That’s why we strive to involve patients and their families in maintaining good health that lasts a lifetime. We’re committed to the health of our communities, and we’re always here to listen, understand and respond to your healthcare concerns. Fort HealthCare Jefferson family practice works to help the entire family achieve total health through prevention, education and, if needed, medical intervention.

Comprehensive Care

A first-time consultation at our clinic involves a complete review of your medical history, with particular attention to your symptoms and concerns, followed by a thorough examination, after which an individualized treatment plan is developed.

To make an appointment, simply call our office and explain the reason for your visit so we may schedule an appropriate amount of time with the physician. Family Medicine is the medical specialty that provides continuing and comprehensive healthcare for each member of the family, over the course of their lifetimes.

At Fort HealthCare, our family medicine physicians and nurse practitioners emphasize preventive medicine and maintaining a healthy lifestyle. Our goal is to help you and your family live healthy, happy lives.

No one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule available based on family size and income.
