  1. The Wisconsin Home Energy Assistance Program (WHEAP) DEHCR Wisconsin Home Energy Assistance Program provides one-time heating, energy bill, furnace and crisis assistance. Eligibility is based on several factors, including income.   1. Wisconsin Works (W-2) is Wisconsin’s primary Temporary Assistance for Needy Families (TANF) program. W-2 is a program that provides employment preparation services, case management and cash assistance to eligible families. W-2 can help you get a job and provide for your family.   1. The Telecommunications Equipment Purchase Program (TEPP) is to help people with disabilities buy specialized equipment they need to use basic telephone services. Applications can be found at https://psc.wi.gov/Pages/Programs/TEPP.aspx. Takes 3-4 weeks to receive a TEPP voucher.
  2. The Lifeline program provides low-income Wisconsin residents affordable access to essential telecommunications by discounting the cost of phone, cell and internet services.   2. Mobility Transportation Management provides rides for $10. Co-pay is requested for round trip out of county ride and $2 co-pay for a round trip in county ride. Non-Emergency Medicaid and BadgerCare Appointments. Rides to routine appointments may be scheduled from 7:00 a.m. to 6:00 p.m. Monday – Friday. Located at 1541 Annex Road, Jefferson.   3. Veteran’s Van – The Veteran’s Administration has provided Jefferson County with a van to take vets to medical appointments at the VA Medical Center in Madison.   4. Mobility Managers – Mobility Managers help older adults, people with disabilities and anyone facing barriers finding transportation. Mobility Managers focus on meeting individual customer needs through a wide range of transportation options and service providers. Ride United – Free transportation to and from local food pantries in Fort Atkinson, Jefferson and Whitewater as well as the Rock River Free Clinic, Community Dental Clinic and the Winther Counseling Lab. Please remember the Colonoscopy is still the preferred recommendation from Fort HealthCare. A colonoscopy is the best colorectal cancer screening test because it also allows your doctor to prevent colorectal cancer. Other non-colonoscopy screening tests look for signs of blood, and in some cases, DNA markers, in your stool that can be signs of colorectal cancer.

  4. Blue Hope Financial Assistance: The Blue Hope Financial Assistance program is a multifaceted program that provides assistance for colorectal cancer screening and treatment. The COVID-19 crisis and changing economy has increased financial anxiety and needs across the country. If you are a colorectal cancer patient in treatment, you are eligible to apply for our crisis financial awards. If you are seeking colorectal cancer screening, such as colonoscopy, we can help connect you to a resource that works best for you. Funds may be used for any daily living expenses such as household bills, childcare, transportation costs, or food. Applicants who are U.S. residents and are eligible may receive a $200 award.   1. The Fort HealthCare community mammogram program was implemented to make sure that all women that needed a mammogram had the opportunity to receive one. The ongoing effort also extended mammogram appointments into evenings and weekends. Dozens of free mammogram vouchers have been made available to underinsured or uninsured women in the community. Fort Healthcare offers Eye Screening with a teleophthalmology. Teleophthalmology is a low cost, convenient method in which a picture of a patient’s eye is sent to an ophthalmologist offsite to diagnose then the results are sent back to Fort HealthCare and then will be communicated to the patient. The cost is less than $40 and there are free vouchers available at the clinics upon patients need. Fort HealthCare is partnered with the Lion’s Club to help cover cost of the eye exam for patients in financial need.

Eye Care America: Public service program that provides comprehensive eye exams and eye care at no cost to eligible candidates aged 65 or older and people at increased risk for glaucoma who qualify for the program for up to one year.   4. New Eyes for the Needy Application: New Eyes provide a basic pair of single or lined bifocal lenses. Special requests including upgrades such as tinted, progressive or Transition lenses are beyond the scope of New Eyes’ program. A New Eyes voucher is for individuals who have no other resources with which to obtain a basic pair of eyeglasses. Wisconsin Project: The Wisconsin Project is an independent program run by the Wisconsin Optometric Association (WOA), a non-profit association consisting of optometrists that practice in Wisconsin. The program offers comprehensive eye care services to children aged 18 and under who are from low income, working families and have no insurance which covers vision and eye health care.   2. SeniorCare is a prescription drug assistance program for Wisconsin residents who are 65 years of age or older and meet the enrollment requirements. The program is designed to help seniors with their prescription drug costs. To enroll in SeniorCare you must: Be a Wisconsin resident, be a U.S. citizen or have qualifying immigrant status, be 65 years of age or older and fill out a SeniorCare application. Every senior enrolling in SeniorCare must pay an annual $30 fee. The earliest you can apply is during the calendar month of your 65th birthday. If you are already age 65 or older, you can apply at any time. Your benefits will begin in the month after you apply. Your annual income determines your level of coverage in SeniorCare and how much SeniorCare will cover.
