The Fort HealthCare High-Five program is where employees recognize employees for their outstanding work. Complete the form below to make your nomination.

The High-Five Committee will randomly choose one winner each month for each RESPECT value. If your nominee is chosen, you’ll also be entered to win a prize!

- Responsiveness - examples may include volunteering for projects, communication and follow through without being asked, willing to take on additional responsibilities, consistently stepping in and helping others, and using downtime effectively.
- Excellence – examples may include mentoring others, a leader in managing an appropriate schedule so patient care is not interrupted, willing to share constructive ideas, comments, and concerns during meetings, and offering solutions.
- Sensitivity – examples may include being intuitive to patient and coworker concerns, showing compassion, and being responsive to others’ needs.
- Professionalism – examples may include identifying processes that need improvement and implementing plans, or being reflective to own actions taken, owning those activities, and taking steps to ensure corrective steps are taken for improvement.
- Empowerment – examples may include consistently looking for and identifying ways for team to succeed and offering solutions, embraces new initiatives, supporting Leadership, teaching others, and effectively motivating others to attain organizational goals.
- Cultural Diversity – examples may include handling people and situations with poise and understanding, embracing all backgrounds, differences and beliefs, and effectively turning defensive situations into supportive relationships.
- Teamwork – examples may include being invested in the teams success, finding positive ways to bring the team together, consistently contributing to the success of the team, and excellent communication, sharing of information, and listening to others.
