The 2017 Employee Wellness Program Year is fast approaching! This is a reminder that biometrics, labs, and Personal Health Assessments are available now for ALL FTE (includes Fort Medical Group) employees. Participating in the Fort HealthCare Employee Wellness Program gives you an opportunity to receive participation-based and outcomes-based incentives throughout the year, as well as improve your overall health.
ALL OF THE FOLLOWING STEPS MUST BE COMPLETED BY THE DEADLINES INDICATED TO BE ENROLLED IN THE EMPLOYEE WELLNESS PROGRAM AND ELIGIBLE FOR PARTICIPATION INCENTIVES:
1. Biometrics – February 1st-20th (no exceptions to the deadline) – HOSPITAL BIOMETRICS SCHEDULE – CLINICS BIOMETRICS SCHEDULE
2. Lab work – February 1st-20th (no exceptions to 20th deadline) – Click here to view the recommended LAB SCHEDULE
3. Personal Health Assessment (PHA) – February 1st-20th (no exceptions to 20th deadline) – You can find the link to the PHA at the top of the wellness portal at CERNERHEALTH.COM. You must be logged in as yourself to fill out the PHA. If you do not have a Cerner Health account, please contact Marci at marci.pasquesi@forthc.com for instructions on setting up your account.
4. Advisor Visits – March 1st-24th – If the results of items #1-3 above indicate that you would benefit from a visit with a health advisor, an email will be sent out to you. At that time, you will be able to view a schedule of advisor visit times to register for at CERNERHEALTH.COM. Advisor visits are 30 minute, one-on-one health coaching sessions.
