We previously sent a request to patients to help us identify critical health needs to improve community health and measure the impact of Fort HealthCare’s programs. We are short on responses from individuals 18-44 years old and particulary males. If you have not yet responded, we need your response to ensure we are aware of the health needs of patients like you.
