Fort HealthCare Announces 2025 Patient & Family Advisory Council (PFAC)

FORT ATKINSON – Fort HealthCare is excited to announce the members of its 2025 Patient & Family Advisory Council (PFAC), a group dedicated to ensuring the voices of patients, families, and community members are heard in shaping healthcare services and improving patient experiences.
The 2025 PFAC members include:
- Andy Yocom (Fort Atkinson)
- Delores Burington (Fort Atkinson)
- Nancy Baines (Volunteer – Fort HealthCare)
- Bryan Carrera-Ramos (Employee – Fort HealthCare)
- Britanie Peaslee (Lake Mills)
- Monica Los (Whitewater)
- Lesli Rumpf (Cambridge)
Molly Prater serves as the PFAC Chair.
The PFAC serves as a bridge between the community and Fort HealthCare, fostering collaboration to enhance care delivery, patient satisfaction, and community outreach. By working closely with council members, Fort HealthCare aims to elevate the healthcare experience by integrating feedback and insights directly from those who use its services.
“The Patient & Family Advisory Council embodies Fort HealthCare’s commitment to patient-centered care and meaningful community engagement,” said Molly Prater, PFAC Chair. “We are grateful to have such a diverse group of individuals who are passionate about making a positive impact on healthcare for everyone in our region.”
The PFAC meets regularly throughout the year, addressing key areas of care improvement, patient safety, communication, and overall patient and family satisfaction.
