Fort HealthCare COVID-19 Vaccine Protocol

Fort HealthCare understands that people are eager to receive the COVID-19 vaccine. On Monday, January 25th we will move to the first part of Phase 1B and those ages 65 and up will be eligible for the vaccine, in addition to those who qualify under Phase 1A.
If you are confident that you would like to receive both doses of the vaccine, please complete the COVID-19 interest form online to be put on a waiting list at FortHealthCare.com/VaccineSignUp. After completing the form, eligible patients age 65 and older will receive an invitation to receive a COVID-19 vaccination. While this interest form does not guarantee an immediate appointment or replace the scheduling process, we may contact you for same day appointments if they become available.
Priority scheduling will be given to current Fort HealthCare primary care patients. However, availability is open to the public. To expedite your access to the vaccine it is recommended you sign up for a MyCompass account through self-enrollment which is located on the Fort HealthCare website.
If you do not currently have a primary care physician, another option is the Jefferson County Health Department. They will also be holding vaccination clinics. You can sign up at https://www.surveymonkey.com/r/65EKDISL
Due to the large number of individuals who are eligible and the limited amount of available vaccine, it may still be some time before we are able to schedule everyone for a vaccination. We will be sending communication through MyCompass and will have updated information on Fort HealthCare.com
