The Fort HealthCare Diabetes Education department helps individuals manage their condition through personalized information, support, and education. Alyssa sees patients by appointment at her office at Fort Memorial Hospital. A referral by a healthcare provider and a diagnosis of diabetes is required. Certain health insurances may cover appointments for diagnoses of pre-diabetes.
The Diabetes Education program and curriculum is recognized by the American Diabetes Association and follows American Association of Diabetes Educators guidelines for self-care behaviors.
Abby uses an individualized approach to each person’s diabetes self-management plan. Abby believes a personalized and engaging teaching session empowers people to manage diabetes in their daily lives. “I hope to provide support to people with diabetes. We will collaborate to make personalized goals for you which allow you to make small, sustainable changes.”
“When I am not at work, I am often spending a lot of time with my family. My daughter and 2 dogs keep my husband and I busy! We like to spend time outside when the weather allows. I also enjoy photography.”
