Fort HealthCare Diabetes Education

The Fort HealthCare Diabetes Education department helps individuals manage their condition through personalized information, support, and education. Alyssa sees patients by appointment at her office at Fort Memorial Hospital. A referral by a healthcare provider and a diagnosis of diabetes is required. Certain health insurances may cover appointments for diagnoses of pre-diabetes.

The Diabetes Education program and curriculum is recognized by the American Diabetes Association and follows American Association of Diabetes Educators guidelines for self-care behaviors.

About Alyssa’s practice

Alyssa provides her patients with the encouragement and motivation they need to be able to make small lifestyle changes that will make a positive impact on their health. “It is my goal as the Diabetes Care and Education Specialist, to help people live the best life they can, while managing their diabetes. I love to provide my patients with the tools and resources they need to live a healthy life.”

Personal Interests

“ In my free time I enjoy spending my time outdoors. This may be going rock climbing, camping, or hiking with my dog. During the winter months I enjoy ice skating. My other hobbies include trying new baking or cooking recipes.”
