If you have diabetes, or have been newly diagnosed with the condition, you may be overwhelmed at times with all the information and steps required to monitor and manage your condition. As a member of your healthcare team, a diabetes educator makes managing your diabetes easier. A new diagnosis can bring about all sorts of new feelings, questions, and concerns.
Our providers are available to help you navigate through and help you manage your new lifestyle. Working first with your primary care physician, our diabetes education team is there to provide personalized information, support, education, and tools for success. They work with you to develop a personalized plan to stay healthy, and give you the tools and ongoing support to make that plan a regular part of your life.
Diabetes education is an important part of your diabetes care and is covered by Medicare and most health insurance plans when it is offered through an accredited diabetes education program, like ours. A referral by a healthcare provider and a diagnosis of Diabetes is required. Certain health insurances may cover appointments for diagnoses of Pre-Diabetes.
The mission of the Diabetes Education department at Fort HealthCare is to provide quality comprehensive diabetes self-management education to our community. We believe that education is the key to empowering the person with skills to better manage his or her diabetes, avoid the complications, and achieve an optimum heath status.
Our Diabetes self-management education and curriculum is recognized by the American Diabetes Association (ADA). To be a recognized by the ADA means the program has demonstrated that it meets the National Standards for Diabetes Self-Management Education. These standards include making sure the program is available to all who need it; that the program is supported by an organization providing stability, advancement, and ongoing program coordination; the program is led by trained instructional staff using an approved curriculum and encouraging participation; and that individualization of care plans and ongoing support is provided.
Our program follows American Association of Diabetes Educators guidelines for self-care behaviors, which include healthy eating, being active, monitoring your blood sugar levels and general health, taking any necessary medication, problem solving to help you prepare for the unexpected, reducing risks for developing other health problems, and healthy coping.
Fort HealthCare Diabetes Education & Support Group
This group meets on a monthly basis and is facilitated by a Fort HealthCare Diabetes Educator. Attending the group is free of charge and is a good environment for fostering relationships with others who have diabetes, know someone with diabetes, are curious about diabetes, or have been diagnosed with pre-diabetes. The goal of the group is to provide education and practical tips related to the prevention and management of diabetes. Download the latest flyer for details.
If you are under 18 years old with diabetes, you and your family can apply to join the Fort HealthCare Diabetes Youth Group on Facebook. The group is organized by our diabetes education team, and gets together for educational sessions and fun activities. Families that participate in the program find it very helpful for sharing and learning ways to manage a new lifestyle.
