LeAnn Ozminkowski, PharmD

Medical Practice

LeAnn is a clinical pharmacist at Fort Healthcare, joining the pharmacy team in July of 2022.  She received her Doctor of Pharmacy from the University of Wisconsin-Madison in 2001.  Prior to Fort Healthcare, LeAnn has worked at several other hospitals in various capacities including clinical pharmacist, clinical coordinator, educator on certain topics for both nursing and providers, and interim director.

Personal Interests

LeAnn achieved her board certification as a pharmacotherapy specialist (BCPS) in the fall of 2023.  She has always had a love for rural health and is becoming involved in different committees at Fort Healthcare.  Rural Healthcare has always provided unique opportunities to work with patients and members of the healthcare team.  In her spare time, LeAnn enjoys traveling with her family, snuggling with her two dogs, attending her kid’s sporting events and other activities, and playing volleyball in the summers.
