About Sam

Sam joined Fort Health Care’s Comprehensive Care Management team (CCM) in May 2021. Before joining the CCM team, Sam worked at MyChoice WI (formerly known as Care WI) as a Case Manager. Prior to Case Management, Sam worked at Fort Health Care as a Triage/rooming RN and Resource Nurse at the Internal Medicine and Pediatrics clinic and School Nurse within the Fort Atkinson School District.

Sam sees Comprehensive Care Management patients at the following clinics:

Personal Interests

Sam has a husband and two sons at home who keep her quite busy. At home, you can often find her in the garden or flower beds. Her family enjoys biking, hiking and pretty much anything “outdoorsy”. She looks forward to spending time with friends and family.
