Mammogram Voucher Program

If you are uninsured or a screening mammogram is not covered under your insurance, Fort Memorial Foundation vouchers are available to use for a free mammogram at our hospital. For more information about Fort HealthCare Mammogram Vouchers, please speak with a Fort HealthCare Primary Care clinic or the Rock River Community Clinic.

If a diagnostic mammogram or other services are necessary after your screening mammogram, and you are under or uninsured, please contact one of our Patient Financial Counselors at 920-568-4722 to determine what Financial Aid may be available to you.
