Mammogram Voucher Program

If you are uninsured or a screening mammogram is not covered under your insurance, Fort Memorial Foundation vouchers are available to use for a free mammogram at our hospital. For more information about Fort HealthCare Mammogram Vouchers, please speak with a Fort HealthCare Primary Care clinic or the Rock River Community Clinic.

If a diagnostic mammogram or other services are necessary after your screening mammogram, and you are under or uninsured, please contact the Business Office to determine what Financial Aid may be available to you.
