The Community Health Needs Assessment (CHNA) is a comprehensive, data-informed process designed to identify the most significant health needs of Jefferson County residents. Through systematic data collection, thoughtful analysis, and coordinated community input, the CHNA helps establish shared priorities and guide meaningful health improvement efforts.
Every three years, Fort HealthCare collaborates with regional partners as a member of the Dodge Jefferson Healthier Community Partnership to complete the CHNA. Community perspectives are gathered through focus groups and a community health summit, ensuring that the voices, experiences, and health needs of Jefferson County are heard, understood, and acted upon.
The resulting Community Health Improvement Plan (CHIP) then transforms these priorities into action by outlining focused strategies, collaborations, and measurable goals aimed at improving health outcomes across Jefferson County.
