Eligibility for the following benefits is dependent on your FTE:
Health, Dental and Vision Insurance – Our health plan is three-tiered utilizing Fort HealthCare Providers, as one of our In-Network options; we also offer an In-Network option through UHC PPO, and have coverage for Out-of-Network Providers. Our dental plan allows you to go to any dentist you would like. Vision is also available as a separate plan.
Life and Accidental Death and Dismemberment Insurance – Fort HealthCare provides, at no cost to you, Life/AD&D coverage at two times your annual salary. Long Term Disability Insurance – Fort HealthCare provides, at no cost to you, LTD coverage. Short Term Disability Insurance – Fort HealthCare provides, at no cost to you, STD coverage for up to 13 weeks for illness or injury, including maternity, at 70% of your pay.
Retirement Plan – Available upon hire, Fort HealthCare offers a 401(k) plan with pre-tax or post-tax contribution options. Fort HealthCare will match your contribution percentage up to a maximum of 4%. The 401(k) plan is offered to all employees age 21 years or older. Employer and employee contributions are 100% vested from the first dollar.
Flexible Spending Plan – Fort HealthCare health and dental insurance premiums, reimbursable medical expenses and reimbursable dependent care expenses can be deducted pre-tax.
Employee Assistance Program – Available to employees and their spouses and dependents. Our EAP plan provides an initial confidential consultation and assessment paid for by Fort HealthCare.
Educational Benefits – Fort HealthCare provides a Tuition Reimbursement Program available after six months of employment, a Career Ladder Program, and reimbursements, upon hire, for Educational Programs directly related to your employment.
Other Benefits – Fort HealthCare provides a generous Paid Time Off plan; first year Full-Time employees can accrue up to 24 days of PTO annually. Fort HealthCare offers many other employee benefits including pharmacy discounts at our in-house pharmacy, a wellness program, on-site daycare and more.
