Interested Applicants Must:
- Be a graduate from an ACPE-accredited College of Pharmacy
- Be eligible for licensure in the State of Wisconsin
- Participate in the ASHP Resident Matching Program
- Submit the following residency application materials through PhORCAS
- Basic demographics
- Verified transcripts
- Curriculum vitae
- Extracurricular information
- 3 letters of reference (at least one must be from an APPE preceptor)
- Personal statement/Letter of intent (Specific information about practice goals and areas of interest; what interests the candidate most about our program; and how doing a residency will help the candidate accomplish their career goals)
- Participate in requested site interview
