David Dulak, PharmD, BCPS

Professional Practice

David is a clinical pharmacist at Fort Healthcare. He received his Doctor of Pharmacy from the University of Wisconsin-Madison in 2016, and completed a general practice PGY-1 residency at Gundersen Lutheran Medical Center in La Crosse, Wisconsin. Before graduating from pharmacy school, he worked as a pharmacy intern for Walgreens. He also worked as a pharmacy technician for UW Health in Madison and Mayo Clinic Health System in Red Wing, Minnesota. He maintains membership in the American Society of Health-System Pharmacists, American Pharmacists Association, Pharmacy Society of Wisconsin, and Phi Delta Chi Professional Fraternity.

Personal Interests

David’s clinical interests include critical care/emergency medicine, behavioral health, infectious disease, and compounding. He enjoys teaching and working with pharmacy residents and students, especially in a rural health setting. In his free time, David enjoys playing volleyball, racquetball, poker, video games, taking naps, and playing drums with a country western band based out of La Crosse, WI.
