Fort HealthCare Jefferson Clinic Construction Completed

Jefferson ClinicThe newly remodeled Fort HealthCare Jefferson clinic opened in August at its permanent location, 840 West Racine Street in Jefferson. The new clinic is a modern space capable of supporting up to three full time providers practicing at the same time and has a procedure room, new imaging equipment and a small lab area on-site for extra convenience. The clinic’s construction was managed by Stevens Construction Corp., which uses a variety of subcontractors that are specialized in certain areas. Every effort was made to use local vendors and contractors when possible.

Linda Welniak, MD, Noreen Lyons, APNP and Joshua Tueting, APNP continue to provide care for Jefferson and other area residents at the clinic. All providers are accepting new patients, and same day appointments are available for urgent care issues. Individuals may make an appointment at the clinic by calling 920-674-6000.

The Jefferson family medicine clinic provides routine care and coordinates specialty care for people of all ages – from infants to adulthood. Welniak was born and raised in Waukesha, trained at the Medical College of Wisconsin, and brings 16 years of experience practicing in both Milwaukee and Waukesha. Dr. Welniak has a passion for all aspects of Family Medicine from pediatrics through adult medicine and is very happy to be caring for patients in the Jefferson area.

Tueting is a graduate of Marquette University with a Master of Science as an Adult Primary Care Nurse Practitioner. As an adult nurse practitioner, he works with patients fifteen years of age and older, focusing on health maintenance and improvement for patients.

Family nurse practitioner Lyons has been providing care in the community for many years, having studied at the University of Wisconsin-Milwaukee and receiving certifications as a pediatric nurse practitioner and family nurse practitioner from the American Nurses Credentialing Center. She has special interests in family and pediatric medicine, promoting health lifestyles and educating patients about how to live healthier lives.
