Fort HealthCare Jefferson patient services move to Rockwood Clinic

Effective Thursday, November 10, all patient care activities of the Fort HealthCare Jefferson location will be provided at the Fort HealthCare Rockwood location, 152 W. Garland St., Jefferson. This includes the medical care services provided by Dr. David Rutledge and nurse practitioner Noreen Lyons. Both clinic locations will be closed on the Tuesday and Wednesday prior to that date to allow for the move. Patients requiring assistance during that time can continue to call the clinic at (920) 674-6000.
The Fort HealthCare Jefferson location will undergo extensive physical changes. A new clinic facility will be constructed on the site of the current building located at 840 West Racine Street. The remodeled clinic will be a modern space capable of supporting up to three providers practicing at the same time. It will also house a procedure room, new imaging equipment, a small lab area, and enhanced nursing and reception areas. The timing for this re-build of the clinic is yet to be finalized, but a spring or summer 2012 occupancy is anticipated.
Also effective with this change, family physicians David Misorski and Geoffrey Smith, who have been practicing at the Rockwood location in addition to their regular practice at Fort HealthCare Johnson Creek, will discontinue seeing patients in Jefferson. Patients can reach them at (920) 699-4000. The Fort HealthCare Johnson Creek location is at 400 Doctors Ct.
Rutledge has accepted an appointment as the full-time medical director at the Rock River Free Clinic, an appointment that begins in January 2012. This commitment helps the Rock River Free Clinic board reach its goal of adding more regular physician services to this invaluable program.
Coincident with Rutledge’s departure, Fort HealthCare welcomes family practice physician Linda Welniak, M.D. Welniak joins the Fort HealthCare staff on January 3rd. Patients can schedule appointments by calling (920) 674-4141.
