Fort HealthCare is pleased to welcome a new family nurse practitioner to its team of providers. Rachel Lee, APNP joins David Misorski, MD at both the Fort HealthCare Jefferson and Johnson Creek clinics, and also Miranda DeThier, PAC in Jefferson. The Jefferson clinic is located at 840 West Racine Street in Jefferson, and the Johnson Creek clinic is located at 400 Doctors Court in Johnson Creek.

Rachel received a Doctor of Nursing Practice from the University of Mississippi and a Bachelor of Science degree in Nursing from the University of Delaware. She is Board Certified by the American Association of Nurse Practitioners.

She enjoys providing care on behalf of Fort HealthCare because of its strong community presence. “I am passionate about preventive medicine and wanted to be a part of an organization that not only values illness management, but strives toward sustaining wellness. I am excited to be a part of the team working to create the healthiest communities in Wisconsin.”

Rachel views the practice of medicine as a partnership between provider and patient. “It is my job to present treatment options based on the best evidence and to help people develop a plan that considers their current lifestyle and goals. I also encourage all of my patients to become knowledgeable about their health and I am happy to answer their questions.”

“In my spare time, I enjoy spending my free time outside, especially running, hiking, and swimming. I like participating in running and triathlon events as a way to explore new places. I also enjoy traveling, cooking, and reading.”

Primary care and family medicine providers are medical professionals that are a person’s main healthcare provider in non-emergency situations. They are trained to treat common conditions and to direct their patients to specialists and coordinate that care when needed. Since your primary care provider cares for you through several stages in your life, they get to know you well and may notice changes that are important to detecting health problems early.

Fort HealthCare participates in most insurance plans, including Dean Care, Dean Care Gold, Unity, Physicians Plus, MercyCare, Humana, United Healthcare, and more. For a complete listing, visit FortHealthCare.com/Insurance.
