Fort HealthCare encourages everyone to carry an up-to-date safe medication card with them at all times that can be referenced quickly.

Keeping a list of your medications is a way for you to be more involved with your healthcare, and to safely prepare for medical emergencies. Your list should include prescription drugs, over-the-counter medications, vitamins, supplements, and herbal remedies.

Why It Matters

  • Helps emergency responders and doctors treat you faster
  • Prevents harmful drug interactions
  • Keeps your entire care team—including pharmacists—informed
  • Assists family members filling out your medical history

What to Include in Your Medication List

  • Full name of each medication or supplement
  • Dosage and time(s) of day taken
  • Reason for use
  • Start date
  • Pharmacy contact information
  • Allergies or intolerances (include reaction details)
  • Recent vaccinations
  • Names and contact info for your healthcare providers
