Soak up the season with Fort HealthCare’s and the Jefferson County Health Department’s Sunshine & Strong 6-week community wellness challenge! From June 8 through July 20, this uplifting and energizing challenge is designed to help you make the most of summer by focusing on movement, healthy habits, connection, and fun in the sunshine.
Simply print your challenge calendar, complete daily wellness activities, and track your progress along the way. Turn in your completed calendar for a chance to win fun gift-card prizes! Open to all ages, with activities designed for both kids and adults, Sunshine & Strong is a great way for individuals, families, and friends to build healthy habits together all summer long.

How-To Participate
Participating is easy! Print off your Sunshine & Strong calendar (English or Spanish version) and start tracking your accomplishments on June 8th. Once the challenge ends on July 20th, fill out your information and turn in your calendar to be eligible for prizes.

Goals of the Challenge
- Encourage well-rounded wellness with a focus on movement, mindfulness, nutrition, and connection.
- Promote community engagement through shared wellness experiences.
- Inspire participants to embrace joyful, sustainable habits during the summer months.
Prizes
Prizes will be randomly drawn the week of July 27th. The more spaces you cross off on your calendar = more entries to be eligible for winning!
Adults: 10 x $25 gift cards, randomly selected from those that turn in calendars
Kids: The first 30 kids that turn in a calendar will receive a FREE pint of Culver’s ice cream (Redeemable at the Fort location only)
Entry System
- 14-20 boxes checked off = 1 entry
- 21-27 boxes checked off = 3 entries
- 28-34 boxes checked off = 5 entries
- 35-42 boxes checked off = 10 entries
Calendar Drop-Off Locations
Once your calendar is completed and dates are marked off, fill out the information on the bottom right of the second page of the calendar and turn in the following ways through July 26th:
In-Person
- Fort Memorial Hospital Front Desk (611 Sherman Avenue East, Fort Atkinson)
- Jefferson County Health Department (1541 Annex Road, Jefferson)
Online
- Scan-to-email OR email a picture of your completed calendar to wellness.advisor@forthc.com
