Fort HealthCare and Rainbow Hospice offers free advance care planning to help you understand possible future health choices and make a plan for future health care situations. Fort HealthCare and Rainbow Hospice have facilitators available to meet with people on an individual basis or to speak to your organization or group as a whole on the importance of advance care planning.

Fort HealthCare’s medical social worker, Lori Partoll, will be at the Internal Medicine Office at 500 McMillen St. on April 16th from 9:00 a.m. until noon to provide advance directive forms and assist with making an appointment with a trained facilitator who can guide individuals through the process of advance care planning.
