Area Medical Providers: Meeting Your Medical Needs

There are more than 100 outstanding physicians on the Fort HealthCare medical staff. Physician specialists include primary care physicians and specialty care physicians. Fort Medical Group is a multi-specialty group practice with satellite clinics offering primary and specialty care in Fort Atkinson, Cambridge, Jefferson, Johnson Creek, Lake Mills and Whitewater. Our Specialty Clinic invites physicians from these partner organizations and more to treat patients close to home. These much-needed experts are available by appointment throughout the week.

There are also primary care clinics in Whitewater and Fort Atkinson served by Dean/St. Mary\’s Regional Clinics, UW Health and the Rock River Foot & Ankle Clinic. The mission of the Rock River Free Clinic is to provide access to free medical services through health professionals dedicated to caring for low income and uninsured individuals. Medical and radiation oncologists work closely with Fort HealthCare clinical staff at the UW Cancer Center Johnson Creek. This unique partnership with the University of Wisconsin Carbone Cancer Center provides exceptional care close to home.

Fort HealthCare believes that an active lifestyle, regardless of one’s physical abilities or limitations, is the key to good health. In keeping with our Mission to improve the health and well being of our communities, we sponsor a myriad of community events. Sponsorships are awarded to those organizers who share our mission and offer wellness events such as runs, walks and bike rides. A few of the beneficiaries are Tomorrow’s Hope, the Jefferson County Literacy CouncilPADA, high school booster clubs, the Boys & Girls Club and the Fort Half Marathon.

Our commitment to active lifestyles is demonstrated by the Fort HealthCare sponsored Health365Events website – where local organizations can post and share local health and wellness events.  This website fosters a greater sense of community health and proves that being healthy and active should be fun.

Fort HealthCare’s commitment to reducing obesity and providing access to food sources that offer nutritious products is shown by our support of community supported agriculture (CSAs). Fort HealthCare is an annual sponsor of the Fort Atkinson Farmers Market, The Lake Mills Farmers Market and the Jefferson Farmers Market. Fort HealthCare is also a sponsor of the Farm Fresh Atlas of Southeastern Wisconsin. Locally sourced foods are featured every day at Fort Memorial Hospital\’s Steel Away Café and on the trays delivered to patients. Wholesome, fresh foods are essential for maintaining a healthy workforce and are a treat for our patients and visitors.

Fort HealthCare has entered into a strategic partnership with Cerner Corporation to implement a comprehensive electronic health record (EHR) system known locally as Compass. The rationale for installing more than 50 Cerner Solutions is summed up in these four simple statements:

The Compass EHR provides physicians, nurses, pharmacists and lab technicians access to one patient record all at the same time. This access to relevant, integrated clinical information allows medical professionals to communicate instantly with the rest of the patient’s care team and enables them to make the best care decisions possible.

The MyCompass patient portal allows Fort HealthCare patients to direct, manage and monitor their health, and their family’s health – easily, immediately, accurately and securely via 24/7 online access.

Obesity, diabetes, tobacco and alcohol abuse and other serious health problems are prevalent in our communities. With that in mind, Fort HealthCare leadership has recognized the need to take a more future-oriented approach to managing the health of our local population and a more pro-active stance on improving the economic and social welfare of the region.

According to the Health of Wisconsin Survey, residents of Jefferson County have moved from being 28th in 2013, to become the 8th overall healthiest county among the 72 Wisconsin counties .  As Jefferson County’s premier provider of health services, Fort HealthCare is best able to positively impact the overall health of area residents.

Rainbow Hospice Care is a community based non-profit hospice organization that helps terminally ill persons and their loved ones maintain quality of life even at end of life-offering compassionate, expert care that focuses on comfort, dignity and peace.

Fort HealthCare has partnered with Rainbow for 20-plus years, helping our community care for the most serious illnesses and the sometimes-inevitable consequences that occur. We are extremely pleased with the new inpatient center in Johnson Creek, and that the organization continues to serve so many of our residents in their homes.

Residents in the Fort HealthCare service area are fortunate to be able to rely on many high quality providers of long-term care, should that need exist. Long-term care facilities that work closely with Fort HealthCare are Fairhaven in Whitewater, Dycora (Golden Living) in Fort Atkinson, Willowbrook in Lake Mills,  Alden Estates of Jefferson and the specialty care provider St. Colletta’s in Jefferson. St. Coletta’s offers residential and vocational programs that embrace the concepts of independence, community inclusion, choice and empowerment.

Parks & Recreation

A number of local communities have active Parks and Recreation departments. They offer many activities to help you stay active, enjoy the out-of-doors and make new friends.

The Cambridge area is fortunate to be surrounded by many parks, a bike trail and beautiful scenery. Recreational opportunities abound and many activities are organized by the Cambridge Activities Program. Jefferson citizens keep actively involved in more than 55 clubs and organizations. The Jefferson City Recreation Department offers over 40 year-round programs for youth, adults and senior citizens. The mission of the City of Whitewater Parks and Recreation Department is to provide exceptional park and recreation services that enhance the liveability of the community. The Fort Atkinson Parks and Recreation Department offers comprehensive programming for adults and children, and operates the Aquatic Center.

Fort HealthCare views our local schools as key partners in shaping healthy behavior among the area’s youngest residents. For many years, the Fort HealthCare School Nurse program has offered low cost, contracted care for kindergartners through high school students. Nurses provide education about health and wellness to school-age children, school staff and our community, and have access to all the resources that Fort HealthCare has available.

Fort HealthCare athletic trainers similarly provide contracted care to many area high school athletic programs. The trainers and volunteer Fort HealthCare physicians offer training tips, injury prevention and therapy as well as emergency care on the field of play if needed. The Healthiest Community

The mission of the Jefferson County Health Department  is the primary prevention of premature death and disability. The department also works to assure that conditions exist within the county under which people can be healthy. It has responsibility for the Women, Infants and Children (WIC) program to promote and maintain the health and well-being of nutritionally at-risk pregnant, breastfeeding and postpartum women, infants and children.

The Wisconsin Well Woman Program (WWWP) is another important service coordinated by the department. The program provides preventive health screening services to women with little or no health insurance coverage. Well Woman pays for mammograms, Pap tests, certain other health screenings, and multiple sclerosis testing for women with high risk signs of this illness.

The department also operates the Rock River Free Clinic. The clinic\’s mission is to provide access to free health services through health professionals dedicated to caring for low income and uninsured individuals. Health care services are provided to persons within specific income requirements and who do not have health insurance. Fort HealthCare sponsors much of the activity at the clinic and regularly provides volunteer staffing, radiology and laboratory services support.

The Community Dental Clinic is also supported by the Jefferson County Health Department and Fort HealthCare. The clinic\’s mission is to provide basic dental health services to uninsured and Medical Assistance patients in Jefferson County Communities. This six-chair clinic features new equipment, on-site digital x-rays and computerized patient records. Services include examinations, x-rays, routine cleanings, fillings/restorations, extractions and more.

The Walworth County Health and Human Service Department administers and coordinates services for residents to promote health, to prevent and control disease, and to provide health education. In addition to the department\’s responsibility for WIC and WWWP, staff also provides immunization clinics, health screenings, lead poisoning intervention and testing, newborn visitations, tuberculosis control and monitoring and more.

The many residents of Cambridge are served well by Fort HealthCare Cambridge Clinic, where doctors and staff work to help the entire family achieve total health through prevention, education and, if needed, medical intervention.

Cambridge prides itself in offering something old and something new. Victorian storefronts and century-old buildings house shops and galleries. The community has a forward thinking perspective on recreation, education and economic development. The Cambridge area is surrounded by many parks, a bike trail and beautiful scenery. Recreational opportunities abound and many activities are organized by the Cambridge Activities Program.

With nearly 1,000 students, the Cambridge School District serves children and teens from the village of Cambridge, village of Rockdale and parts of the seven townships. Students of all ages benefit from high quality education, low class sizes and a skilled, dedicated professional educational team.

Fort Atkinson

Fort Atkinson is home base for Fort HealthCare, a progressive organization that provides quality healthcare to the residents of an eight-community area. Fort Medical Group is a multi-specialty group practice with satellite clinics offering primary and specialty care in Fort Atkinson, Cambridge, Edgerton, Jefferson, Johnson Creek, Lake Mills and Whitewater. Physicians from Fort Medical Group are joined by those of UW Health, Dean/St. Mary’s Regional Clinics and independent practitioners that bring additional big city medicine to our small town lifestyle. Physician specialists visit the Fort Memorial Hospital Specialty Clinic on a regular basis.

The city of Fort Atkinson is set against the beautiful backdrop of Lake Koshkonong, the Rock River and the Bark River. It is home to more than 12,000 residents. The Fort Atkinson Parks and Recreation Department offers comprehensive programming for adults and children, and operates the Aquatic Center. In all, Fort Atkinson features 13 parks with over 175 acres of park land. The School District of Fort Atkinson is committed to delivering the quality opportunities and services each student needs to achieve his or her academic and personal potential. The district operates four elementary schools, a middle school and a high school.

Fort HealthCare partners with many non-profit agencies that organize events in the community, including United Way of Jefferson and North Walworth Counties, the Boys and Girls Club and the American Heart Association. We sponsor many local events such as the Fort Farmers’ Market and the Fort Half Marathon.

Fort HealthCare provides high quality healthcare to the residents of an eight-community area.   Fort Memorial Hospital is a modern, fully accredited 82 bed facility with more than 100 physicians on staff.  Our primary service area includes neighbors living in Fort Atkinson, Johnson Creek, Jefferson, Whitewater, Lake Mills, Cambridge and Palmyra.

In addition to comprehensive inpatient services, Fort HealthCare offers ambulatory surgery and specialty clinics, a birthing center, cardiac rehabilitation, emergency services including the Urgent Care Program for after-hours treatment of minor illnesses and injuries, occupational medicine and more.

The staff, partners and physicians of Fort HealthCare are resolved to strive endlessly to achieve the promise of both the Mission and Vision statements. We believe both will be achieved by our continuous focus on our Core Commitments.

Jefferson

Jefferson

Jefferson is served by Fort HealthCare Jefferson Family Practice. Staff works to help the entire family achieve total health through prevention, education and, if needed, medical intervention.

Known for its historic buildings and German heritage, the City of Jefferson owns and maintains eight parks, totaling 40 acres of developed open space. An additional 36 acres slated for development will include walking paths, lookouts, playgrounds, open green space and wetland restoration.  The School District of Jefferson  is recognized locally, statewide, and nationally as a district where “student learning and achievement and outstanding character development are paramount”. The district operates a high school, middle school and three elementary schools. The Jefferson City Recreation Department offers over 40 year-round programs for youth, adults and senior citizens.

Johnson Creek

Johnson Creek

Fort HealthCare has made a commitment to the health of the people of Johnson Creek with the presence of the Johnson Creek Family Practice clinic providing continuing and comprehensive healthcare for each member of the family, over the course of their lifetimes. The Fort HealthCare Wound & Edema Center provides expert care in healing problem wounds via specialized care that can speed up healing, reduce additional surgeries, and help patients avoid hospitalization. UW Cancer Center Johnson Creek is a joint community partnership among Fort HealthCare, UW Health Regional Partners- Watertown Regional Medical Center, designed to bring patients and families local access to cutting-edge cancer treatment and research. Services include medical oncology/chemotherapy treatment, radiation oncology and access to clinical trials.

Johnson Creek is known as “The Crossroads with a Future.” The Village’s park and open space system consists of 37 total acres of park land and special use areas. The park system includes neighborhood parks, community parks and special uses throughout the community.

The Johnson Creek School District operates the high school, middle school and elementary school. District staff believes in the potential of all and are committed to developing life-long learners who value themselves; contribute to their community; and succeed in a changing world.

Lake Mills

The physicians at Fort HealthCare Lake Mills Clinic believe in healthcare based on partnership, and strive to involve patients and families in maintaining good health that lasts a lifetime.

Downtown Lake Mills is the site of Common’s Park. Many 19th century buildings that house unique shops and restaurants line the park on all sides of the square. The Lake Mills Chamber of Commerce promotes the recreational and economic activities of the community. The Lake Mills Area School District inspires students to be responsible citizens with integrity and life-long learners in an ever changing world.

Lake Mills offers extensive recreational, shopping and dining opportunities. The 47-mile Glacial Drumlin Trail, located on the southern edge of Lake Mills, follows the old Chicago and Northwestern railway, and offers recreation for hikers and bikers. Aztalan State Park is one of Wisconsin’s most important archaeological sites- an ancient Middle-Mississippian village that thrived between 1000 and 1300 A.D.

The Rock Lake Activity Center is a health club and community center combined, including a complete cardiovascular center with elliptical equipment, stair climbers, treadmills, exercise bikes and more. Fort HealthCare provides financial and programming support that includes educational activities focused on fitness, nutrition, wellness and exercise.

Palmyra

Palmyra lies in the heart of the Kettle Moraine. Recreational opportunities abound in the area. A thirty-mile segment of the Ice Age National Scenic Trailand are located on Palmyra’s doorstep. The John Muir and Emma Carlin Trails along with other scenic trails are nestled within the heart of the Kettle Moraine.

The Palmyra/Eagle School District serves approximately 1,200 students from pre-school through 12th grade. The Palmyra/Eagle School District covers 80 square miles in Jefferson, Waukesha and Walworth counties and operates the high school, middle school and two elementary schools.

Whitewater

Fort HealthCare Whitewater Clinic provides primary care and other medical specialties, bringing comprehensive healthcare for each member of the family close to home. Whitewater Family Practice, a Dean/St. Mary’s Regional Clinic, also serves this community. The Davis Duehr Dean clinic in Whitewater is a comprehensive eye clinic that offers ophthalmology, optometry, contact lenses and a full service optical store.

The city of Whitewater is home to first-class collegiate level education, exceptional tourist attractions, miles of ski and bike trails in the Kettle Moraine State Forest and a growing community of active residents. The mission of the City of Whitewater Parks and Recreation Department is to provide exceptional park and recreation services that enhance the livability of our community.

Fort HealthCare is proud to partner with a number of community-based organizations similarly interested in improving the health and well being of Whitewater. The Whitewater Aquatic Center hosts Fort HealthCare classes. The Working for Whitewater’s Wellness (W3) coalition seeks to promote physical, mental, spiritual and social wellness. Fort HealthCare is a recognized as a founding member of W3 and provides considerable assistance to the organization. The region is especially pleased that the first class University of Wisconsin-Whitewater is nearby. Fort HealthCare partners with the University for market research, board representation and academic internships. The Whitewater Unified School District operates the high school, middle school and three elementary schools, and the LINCS charter school. The district prides itself on its efforts to build a legacy of excellence, student by student.

Bicycle enthusiasts in Jefferson County and Walworth County have hundreds of highway and byway miles to ride on- mostly with low traffic volumes. The picturesque countryside is lined with streams, farms, lakes and of course, cows.

An abundance of paved and unpaved bicycle trails are available, too. Paved trails include the Glacial River Trail and the Highway 26 Trail. Mountain bike trails are available at Cam Rock Park in Cambridge and in the Kettle Moraine State Forest (Southern Unit) near La Grange.

Fort HealthCare encourages everyone to be active. While fitness opportunities exist across the great outdoors, and can be enjoyed year-round, membership to a fitness club can be motivating and fun. Fort HealthCare partners directly with the Rock Lake Activity Center in Lake Mills and the Whitewater Aquatic & Fitness CenterFort HealthCare offers many classes with a fitness theme. Other area fitness centers include Curves, Snap Fitness, Blackhawk Fitness and more.
